Care Management Supervisor - $15,000 Sign On Bonus available for eligible candidates
PRIMARY FUNCTION:
To provide direct Care Management Services, to a defined patient population, while providing supervisory services to the Case Management & Social Work staff. The Care Management Supervisor will provide on-going support to the department. The department Managers, Directors and Vice President will assist in the direction and education of all phases of the Care Management process. The Supervisor will be held accountable to work in a collegial manner with patients, physicians, staff, consultants and all ancillary departments. Will have the responsibility for the direct oversight/supervision of department-based teams; coordinates special departmental projects to include patient and staff satisfaction, length of stay reduction, and service delivery improvement.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Maintains assignment; supports departmental activities for members of the Care Management team at any campus. Identifies and resolves team issues.
Works with the patient, family, and other members of the health care team to formulate a discharge plan that provides the patient services in the appropriate post-acute care setting.
Gathers and assesses information regarding the patient's physical needs, mental status, family support system, financial resources, and available community and governmental resources.
Utilizes assessment skills to develop a comprehensive care management plan that addressed the patients' identified needs.
Provides PTO, disability time, and FMLA oversight to ensure that staffing is optimal to meet service needs
Provides direct supervision to department staff to include orientation, mentoring, coaching and evaluation.
Evaluates staff development opportunities and competencies and performs management and evaluation tasks for all Care Management staff.
Coordinates and initiates training/orientation programs for staff; coordinates ongoing educational presentations - in cooperation with other members of the department and the Staff Development and Education Committee.
Maintains employee files in cooperation with the department Manager/Director.
Communicates and interacts with internal/external customers of the department, including Information Services, Financial Services, Ancillary departments, Third Party Payers, community social service agencies, health care facilities, CMS/QIO, and liaisons with the Department of Nursing.
Assist in the coordination and schedules meetings to support the needs of staff members and improve department service delivery.
Assists in the preparation, review, and monitoring of departmental budgets in cooperation/coordination with the departmental Manager and Director.
Participates in the development and maintenance of departmental policies and procedures.
Participates in Performance Improvement activities specific to departmental functions, system-wide initiatives and, regulatory guidelines.
Prioritizes incoming requests/needs and forwards to the Manges, Directors or Vice President as needed.
Performs/completes other related duties as requested.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Master in Social Work
Master in Social Work; newly graduated from school with Acute care practicum experience.
Minimum 5-7 year experience in health care environment; with at least 2 years serving in a leadership role
An equivalent combination of education and experience may be substituted.
SPECIAL REQUIREMENTS:
Licensed Social Work credential is required in the State of Delaware
CCM/ACM Certification preferred. If certification not in place, willing to complete within one year of hire.
