Job Description: - Provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs. - Enhance the quality of patient management and satisfaction by promoting continuity of care and cost-effectiveness through case management, utilization review, and discharge planning. - Identify and prioritize patients, reviewing work lists to manage new admissions effectively. - Conduct and document assessments and care plans per departmental guidelines using relevant systems. - Participate in daily care management meetings to provide and receive information on patient progression. - Alert care team to any concerns affecting discharge timelines and readiness, and modify plans accordingly. - Assist with identifying expected discharge dates and complete follow-ups as necessary. - Attend weekly complex care meetings, present on patients, collaborate on problem-solving, and monitor action items. - Discuss obstacles to discharge and psychosocial concerns with the multidisciplinary team and coordinate family meetings if needed. - Educate and coordinate referrals to community resources and post-acute providers as necessary. - Communicate medical milestones to patients and families, ensuring understanding and agreement on discharge plans. - Assess post-discharge needs and coordinate the required authorizations for services. - Participate in medication resource management for non-resourced patients as required. - Delegate administrative tasks to Care Management Assistants and consult with Social Workers or Utilization Managers per protocols.
