Care Manager Job Details Level Experienced Job Location James Street Offices - Syracuse, NY Position Type Full Time Education Level 4 Year Degree Salary Range $48,000.00 Salary/year Travel Percentage Some Job Shift Day/Evening Job Category Professional Services Description Elmcrest Children's Center's Health Home Care Management program currently has a full-time Care Manager position available in the Syracuse office, serving the following counties: Cayuga, Cortland, Herkimer, Madison, Oneida, Onondaga, and Tompkins. The Care Manager provides service coordination for enrolled children and families. They assess the need for services, develop service plans, complete home visits and link the child and caregiver to identified services. The Care Manager assists in stabilizing the child's environment and advocates for the child and family's needs, all within the guidelines established by the Department of Health and the Children's Health Home of Upstate New York. Elmcrest's Health Home Care Management is a service for children who have been diagnosed with serious emotional issues, developmental disabilities, chronic medical conditions, or have experienced complex trauma. The Care Manager serves as a single point of contact to manage, on a continuous basis, all of the unique medical, behavioral, and social needs of families. The program supports children in the least-restrictive home or community setting while linking children and their families to services which provide opportunities for improving health and well-being. Qualifications An individual who is highly motivated, independent, organized, and has strong written and verbal communication skills is required. Candidates must have some afternoon/evening hours available and must be willing to travel. Candidates must have a Bachelor's of Arts or Science with 2 years of relevant experience, OR a license as a Registered Nurse with 2 years of relevant experience, OR a Master's with one year of relevant experience. Candidates must possess a valid NYS driver's license with a good driving record. Candidates must successfully complete all mandatory trainings, background checks, and certifications as required by Elmcrest, the Department of Health and the Children's Health Home of Upstate New York. Physical requirements: ability to safely lift/lower 20lbs with or without assistance; ability to walk unaided for short distance; ability to stand, kneel, bend squat
