Job Description :
Cayuga Counseling Services is seeking a Care Manager for The Youth Services Center (The Haven). The Care Manager will serve as the primary point of contact with the enrolled child and their family to provide ongoing advocacy and support to ensure the care coordination network of providers delivers comprehensive, integrated physical and behavioral health care that recognizes their unique needs, is family-and-youth driven, and supports a system of care that builds upon the strengths of the child and family. The Care Manager is responsible for the following:
  • Outreach and engagement to formally enroll referred individuals into the care management program.
  • Conducts the initial Child and Adolescent Needs and Strengths (CANS) assessment to evaluate the needs and strengths of the child and family.
  • Develops the initial care plan driven by the CANS assessment and updates the care plan based on the changing needs of enrollees. Monitors the care plan to assure adherence to the principles of health home and supports health home policies and procedures.
  • Coordinates and facilitates ongoing Care Plan Team meetings that include the child, family, natural supports and any/all network providers assisting in the delivery of the Care Plan.
  • Assures that necessary supports are in place to assist the child and family in carrying out the care plan inclusive of both community service providers and natural supports as identified by the child and family such as peer and family contacts.
  • Collaborates with all services providers establishing both a team communication plan and a crisis plan for the child and family.
  • Provides ongoing monitoring of Care Plan goals, maintains current care management documentation and information regarding care management activities.
  • Completes referrals and follows up as needed.
  • Attends weekly case review meetings.
  • Is a member of the Multi-Disciplinary Team, attends meetings and provides information as requested.
  • Maintain timely and appropriate documentation and record keeping.
Knowledge, Skills, and Abilities:
  • Associates Degree in Human Services with at least Two years of experience working with children or Bachelor's Degree with One year of experience.
(Relevant experience includes proficiency in the human services or medical field in the area of mental health, substance abuse, public health, early childhood or established proficiency on an individual applicant basis). The Agency offers:
  • 3 weeks PTO in your first year, 4 weeks in your second year.
  • 48 hours of wellness time annually (accrue 4 hours per month) that can be used any time you feel you need some time for yourself.
