Job DetailsJob Location New York, NY Position Type Full Time Education Level BA/BS Salary Range $45,000.00 - $45,000.00 Salary Job Shift Monday - Friday 9 AM - 5 PM Description The Bridge, an innovative and well-respected behavioral health agency serving adults with serious mental illness, substance use disorders, and co-morbid medical conditions, is seeking a Care Manager to work in our OMH Licensed Mental Health Outpatient Treatment Rehabilitative Services (MHOTRS) program within our SAMHSA CCBHC. The Care Manager is a full-time position with the responsibility of providing support to our MHOTRS staff and clients. Our clients have histories of trauma, homelessness, incarceration, long-term hospitalizations, and are primarily people of color who are victims of systemic racism. We work from a Harm Reduction, Trauma-Informed perspective and acknowledge the impact that systemic racism has had on the communities we serve. Essential Position Functions:
  • Provide a full array of care coordination services including but not limited to: benefits assistance, linkage to primary health care, housing, and other support services in the community for individuals receiving services at the MHOTRS.
  • Coordinate and liaise with existing service providers, including Care Coordinators and staff in residential facilities, as well as with family members, where consent is provided.
  • Work closely with Fee-for-Service Clinicians to identify areas of need with their clients.
  • Make referrals for Health Home Care Coordination for ongoing services and support.
  • Complete NOMS assessments for the SAMHSA CCBHC.
  • Assist and encourage individuals to develop natural support and include this support in Treatment Plans and client-related meetings.
  • Work with clients, families, and staff in a caring and respectful manner, with due understanding of and consideration for cultural differences.
  • Accompany clients as needed on appointments in the community.
  • Complete required documentation for every client contact in a timely and accurate manner, as per agency policy.
  • Ensure documentation meets regulatory guidelines for level of care.
  • Assist clients in accessing transportation to appointments, and escort clients to appointments.
  • Immediately report serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors.
  • Attend staff meetings, supervision, and training as required.
  • Other duties as may be assigned.
QualificationsQualifications, Skills, and Knowledge:
  • Bachelor's degree required.
  • Minimum 2+ years' experience working with individuals with behavioral health diagnoses, substance use disorders, and trauma histories.
  • Experience using Electronic Health Records.
  • Team player who is flexible, creative, and open to feedback.
  • Bilingual (Spanish) is highly preferred.
