As part of the Maimonides Medical Center Care Management team, you will join a team of dedicated professionals engaged in the rewarding work of providing compassionate care to a diverse population. We currently seek qualified Care Managers to have day-to-day responsibility for coordinating the activities of the care team for patients with social needs, complex medical and/or psychiatric co-morbid conditions and for facilitating patients' access to the full range of medical and psychosocial services in an efficient and effective manner.
In addition, you will:
· Work closely with the interdisciplinary care team including PCP, psychiatrist, therapist, residential services, substance abuse treatment program, social service providers, etc.
· Work closely with the Care Navigator to ensure the flow of information across and between the care team is optimized.
· Provide input to providers/patient/family for written individualized care plans
· Review patient intake assessment and use results to coordinate the completion of the care plan, self-management goals and strategies; document in EMR
· Evaluate medication compliance and assess potential barriers to adherence; ensure medication reconciliation is current
· Receive alerts to inpatient and ER admissions of targeted patients, visit patients during inpatient stays and participate actively in discharge planning and care transition activities; contact patients on the day of discharge
· Outreach to patients to facilitate keeping scheduled appointments; arrange for metabolic and periodic preventive screening
· Provide provision of self-management/ wellness education, peer and other support groups in the language family prefers.
· Organize and participate in case conferences on a periodic basis.
· Review benefits, entitlements, housing with the patient/family and assist in the application process.
· Conduct regular visits with patients in the community including home visits, escorts to medical and behavioral health appointments,
We require:
· BA/BS degree
· 2-5 Experience in healthcare, social work, social services, case management, discharge planning or third-party experience preferred, but not required.
· Experience working with individuals with mental illness, substance use disorders, and/or chronic medical conditions
· Computer literacy include word processing, spreadsheet and database programs
· Excellent English verbal and written communication skills
· Excellent interpersonal, problem-solving and decision-making skills
· Field work required.
· Bilingual preferred: Urdu/Bengali/Spanish/Fujianese
Grant funded position. SALARY: 33.42 - 37.64 based on experience. We are an approved site for NYS social work continuing education credits.
