Coleman Care -
The In-Home Aide will be providing personal care and related services in the home.
He/she provides direct services as outlined in a plan of care for each individual needing help in order to live in his/her own home, and to work closely with the other home care team members.
Responsibilities/Activities:
- Provides those services listed on the Plan of Care developed by the licensed professional which may include, but not be limited to, complete bed bath and totally dependent transfers.
- Organizes tasks in a safe and efficient manner with indirect supervision.
- Reports changes observed in the patient's mental or physical condition to his/her immediate supervisor by phone or in person.
- Assists clients with personal health care tasks as assigned, including bathing, dressing, shaving, shampooing, care of mouth, skin and nails.
- Assists with exercise prescribed and supervised by appropriate professionals.
- Assists with safe ambulation/transportation.
- Positions patient and assists with transfer activities – in and out of bed, chair to walker, etc. as directed by supervising professional.
- Assists patients with toileting when necessary – bedpan, commode or bathroom.
- Understands/recognizes: the importance of nutritious meals and assist with feeding when necessary, the importance of following a special diet when prescribed, and encourages the client to follow the diet plan closely. Performs light meal preparations as directed by professional.
- Takes temperature, pulse, respirations, BP when requested.
- Assists with other procedures, i.e., non-sterile dressing changes, diabetic urine, and specimen collection as instructed by RN.
- Arranges diversional activities when appropriate encouraging patient's mental alertness.
- Performs other duties as assigned by the supervisor.
- Participates in case conferences of patients and their families being served.
- Completes required records for documentation:
- Charting on narrative sheet day of the visit.
- Timely and accurate submission of required documentation of hours worked.
- Participates in training sessions and continuing education as required by the agency to upgrade skills and acquire new ones.
- Assists in maintaining a healthful, safe environment.
- Encourages the patient to become as independent as possible according to the nursing care plan.
- Gives simple emotional and psychological support to the patient and other members of the household.
- Participates in QI activities.
- Complies with agency Infection Control policies.
MACHINES/EQUIPMENT/TOOLS:
Must reliable transportation for work; use of telephone, and appliances in the home.
QUALIFICATIONS:
Must be 18 years of age.Understand and carry out simple directions and instructions.Character and former employer references.Compliance with state and federal employment health regulations.Attendance at Orientation.North Carolina valid driver license.Background check.Able to read, write, and complete simple math problems.PHYSICAL AND SENSORY REQUIREMENTS (With or Without the Aide of Mechanical Devices):
Must be able to move intermittently throughout the workday.Must be able to speak and write the English language in an understandable manner.Must be able to cope with the mental and emotional stress of the position.Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.Must function independently and have flexibility, personal integrity, and the ability to work effectively with the clients, personnel, and support agencies.Must be in good health and demonstrate emotional stability.Must be able to lift weights, etc., not in excess of fifty (50) pounds.Must not present a direct threat to the safety or health of the employee or others.REPORTING RESPONSIBILITIES:
Supervised directly by the Agency Supervisor and Home Care Nurse. Indirect supervision by the Agency Director.
