Job Description :

Under the supervision of the Behavioral Health Supervisor, the Medicaid Assisted Treatment Program Counselor engages patients in the initial and ongoing treatment processes. This role includes conducting preliminary eligibility assessments, preparing intake forms, ensuring access to necessary services within the agency, and making referrals to external resources when needed.

Responsibilities:

  • Patient Sessions: Hold regular sessions with patients to address their problems and psycho-social needs.
  • Case Management: Serve as a Case Manager to coordinate services meeting each patient's assessed needs.
  • Group Therapy: Conduct therapy groups.
  • Problem Identification: Assist patients in identifying and defining their problems.
  • Form Completion: Ensure all necessary forms are completed accurately.
  • Record Maintenance: Maintain accurate and confidential patient records.
  • Training and Credentialing: Attend training seminars on treatment techniques and theories, and obtain the CASAC credential.
  • Data Documentation: Regularly identify and record significant data in case folders and develop treatment plans for presentation to the IDT.
  • Interdisciplinary Collaboration: Hold individual conferences with other disciplines to facilitate effective treatment.
  • Clinical Supervision: Meet weekly with the Behavioral Health Supervisor for clinical supervision and case conferences.
  • Referral Management: Refer patients to other clinic disciplines as appropriate.
  • Feedback Sessions: Periodically meet with other staff to obtain feedback for case management and treatment planning.
  • Forensic Collection: Perform forensic collection for patients on their caseload and monitor treatment-related issues to update treatment plans.
  • Vocational/Educational Assessment: Provide vocational/educational assessment, counseling, and follow-up for patients not assigned to the Vocational Specialist. Complete the Vocational Profile Form (HS 305) for each caseload patient.
  • Vocational/Educational Counseling: Counsel patients on vocational/educational readiness, motivate for vocational rehabilitation, conduct initial evaluations for in-house voc/ed services, and explore voc/ed goals.
  • Referral and Support: Refer patients to the Vocational Specialist and Educational Specialist for in-depth evaluation and counseling. Provide case management, referral follow-up, and supportive counseling for in-house and external voc/ed programs.
  • Consultation and Feedback: Meet periodically with the Vocational Specialist and Education Specialist for case consultation and feedback.
  • Vocational Treatment Objectives: Present vocational treatment objectives at each IDT.
  • Rotational Work: Work on a rotation basis on Saturdays and select holidays, with assigned shifts subject to change based on program needs.
  • Other Duties: Perform other duties as assigned by the manager.
  • Qualifications:

  • Credentialing: Full CASAC (Credentialed Alcoholism and Substance Abuse Counselor) license required.
  • Patient Treatment: Demonstrated ability in delivering patient treatment, including communication skills, specialized Human Services training, understanding of human interaction and personality development, and the ability to write and record case documentation.
  • Regulatory Knowledge: Familiar with regulatory agency regulations regarding patient care operations and practices.
  • Group Dynamics: Essential understanding of group dynamics.
  • Computer Literacy: Proficient in Microsoft Office applications and computerized programs supporting treatment plans and case notes.
  • Communication Skills: Excellent verbal and written communication skills.
  • Team Participation: Ability to participate in various committees and work well in team settings.
  • Language Skills: Bilingual in any language is a plus.
