Job Title: Case Manager RN
Job Type: Contract (4-7 months)
Location: 3 Different Facilities in Camden County
Hours: 8:00 AM - 5:00 PM (Monday - Friday)
Rate: Competitive and Dependent on Experience
Primary Responsibilities:
- Ensure accurate and complete documentation.
- Communicate and collaborate with the interdisciplinary care team.
- Assist the provider/team with various care coordination activities.
- Assist the provider/team with benefit determination associated with Medicare Part A, Part B, and Part D benefits.
- Collaborate with the primary advanced practice clinician (APC) and facility to identify and respond to patient changes in condition.
- Assist the provider/team with therapy coordination.
- Participate in facility-based meetings.
- Schedule and participate in family conferences, team meetings, and team case presentations.
- Coordinate training, tracking, and compliance with quality measures.
- Assist in creating and conducting in-services for selected audiences.
- Ensure complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
- May write verbal orders from the provider in the chart in compliance with state RN practice laws and individual nursing facility practices.
- Assist in managing members through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
- Assist in managing members during short-term sub-acute rehabilitation episodes.
Required Qualifications:
- Active and unrestricted license in the state of residence, or ability to obtain by start date.
- Associate degree or higher in Nursing (RN).
- Certified in Basic Life Support.
- Ability to complete physical requirements, including lifting a 30-pound bag and performing physical assessments.
- Access to reliable transportation.

Preferred Qualifications:
- 1+ year of clinical experience in practice (long-term care setting preferred).
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of geriatrics and chronic illness.
- Proficient computer skills, including the ability to document medical information with written and electronic medical records.
- Effective communication skills and time management.
- Ability to work with diverse care teams in a variety of settings.
- Access to reliable transportation.
- Ability to travel between facilities.
Certification Required:
- Active and unrestricted license in the state of residence, or ability to obtain by start date.
- Certified in Basic Life Support.
- Associate degree or higher in Nursing (RN).
