Compensation Range: $70,000 - $75,000 yearly commensurate with experience.Overview: The Case Manager Supervisor is responsible for all clinical aspects of the City Sanctuary #2 Program. This encompasses care coordination for our families, performing quality assurance tasks through record checks and data reports, providing direct services, taking the lead with resolving crisis situations, and conducting individual psychosocial, assessment/reassessments, and service plans for the active families.Responsibilities: Ensure that the program is contractually compliant and adheres to DHS (Department of Homeless Services) and NYS OTDA (Office of Temporary and Disability Assistance) requirements.Participate in daily huddles, case conferences, and special events.Ensure that referrals are eligible for the program.Oversee and ensure all initial intakes are completed for all families.Complete quarterly assessments, reassessments, and individualized service plans.Ensure that each resident has a current psychosocial assessment.Take the lead with assisting residents successfully transition into permanent, stable housing.Ensure that all have families have a completed HHA application.Submit internal activity forms for new client admissions, changes in income, and discharges.Take the lead with de-escalation and crisis situations which may arise on-site.Participate in the recruitment of qualified staff, including interviewing, and making recommendations for hire. Monitoring and assisting with the new-hire orientation processes to ensure consistency in training among all clinical providers.Enforce the Residential Code of Conduct, Child Code of Conduct, and Good Neighbor Policy.Enthusiastically participate in advocacy and direct actions to end the twin crises of homelessness and HIV/AIDS. Minimum Requirements Bachelor's degree in social work, psychology, counseling, or related field required, MSW preferred.Minimum of 10 years' experience working within case management services.
