Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a CBA ) between Housing Works and the RWDSU Compensation Range: $25/hr - $28/hr commensurate with experienceOverview: The Creative Arts Therapist/Case Manager provides case management services, supportive counseling, therapeutic services, encompassing substance use counseling, mental health counseling, HIV education and entitlement advocacy to clients of the AIDS Adult Day Health Care (AADHC) program. The Licensed Creative Arts Therapist/Case Manager is also responsible for developing and delivering diverse and meaningful clinical creative arts group interventions.Responsibilities: Manages a caseload of 30 - 40 active clients, not inclusive of staff coverage.Provides case management services including but not limited to: monthly assessments, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, referrals (on-site & off-site,) and discharge planning.Completes initial assessment (4A/4B) and quarterly reassessments on all enrolled clients in a timely manner in order to develop appropriate treatment goals for each client, while ensuring timely submission of clinical documentations to managed care organizations for uninterrupted services.Establishes treatment goals and objectives with clients as part of the interdisciplinary team, while ensuring timely submission of clinical documentations to managed care organizations for uninterrupted services.Facilitates up to 6 groups per week, attend all trainings, staff meetings, care plan sessions, and daily morning rounds, performs lunch monitoring for clients at least once per week, accompany client trips/outings, and participates in agency advocacy events devoted to HIV/AIDS.Provides crisis intervention services and, if necessary, refers clients for psychiatric assessments. Consults with supervisor and completes incident reports on all incidents including trauma or behavioral problems requiring AADHC intervention.Coordinates the provision of multidisciplinary services as outlined in interdisciplinary care plan. Periodically reviews clients' Medicaid status and provide spend-down letters if appropriate.Identifies clients receiving services from other community providers and assumes role of lead case manager. Coordinates joint services and treatment planning, assessment and reassessment. Coordinates joint case conferences with other service providers to clarify and define each provider's goals and objectives.Develops a collaborative relationship with clients to monitor integration in the AADHC, to encourage client attendance and provide on-going clinical services. Addresses issues and concerns which may arise for the client during the AADHC participation.Presents psycho-social needs, strengths and limitations of all assigned clients during interdisciplinary care planning meetings and case conferences, as well as provides clinical support to other Case Managers regarding all AADHC clients.Reports weekly updates of all clients in caseload during rounds and discusses outreach efforts for clients with whom no face-to-face contact has occurred.Participates in continuing quality assurance and quality improvement committee activities to ensure the highest level of services to all enrolled clients, along with assurance of documentation compliance related to chart audit and managed care service authorizations.Other duties as assigned by the Clinical Supervisor/Program Director Services or Vice President of AADHC.Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS. Minimum Requirements All applicants must possess a master's degree in creative arts therapy with a limited permit and moving to be licensed in NYS to practice Creative Arts Therapy (LCAT.)Candidate should have at least two years of experience in providing case management services to HIV/AIDS infected substance using population.Skills in group facilitation and counseling are preferred.The ideal candidate should be able to employ a variety of clinical techniques, theories and skills to elicit sensitive information, handle crises and work effectively in an interdisciplinary team.Strong organizational skills and proficient in Spanish are highly preferred.
