Leadership: Assist in the leadership of the residential team, as a member of the management teamParticipate as a member of the management team to ensure the adequate and relevant discussion and communication of individual progress, needs, concerns, etc. to other members of the management team and to inform the team of decisions that pertain to the individualsAttend all required team, management, and campus-wide meetingsParticipate and support activities as necessary and assignedProvide specialized training to supporters, supervisor I's and II's, and residential coordinators in the areas of advocacy, case management and care records maintenanceReporting and Notification: Report all therapeutic physical interventions to families and relevant external agenciesReview and coordinate all incident reportsFacilitate critical notifications and related documentation using Crystal Springs' form and protocol, in addition to systems and formats required by specific funding agenciesEnsure completion and appropriate documentation and filing of all paperwork required of funding agenciesComplete and submit Case Manager Weekly Report reflecting specified task completionsCompile and submit monthly Individual Critical Information report listingReport all anticipated changes in funding as soon as indicated including approved 1:1 staffing to Director of Adult Services and the Chief Financial OfficerFacilitate critical notifications and related documentation using Crystal Springs' form and protocol, in addition to systems and formats required by specific funding agenciesFamily Work: (includes substitute families / guardians, as appropriate).Identify and communicate family concerns, fears, or resistance to treatment to the treatment team members, and assist in resolving related issuesCoordinate, document, and communicate family visitsCoordinate and communicate approved visitor, phone, and mail contactsDistribute weekly visitation schedulesIndividual Record and background information: Collect and compile all pertinent reports and data on individuals on caseload as related to areas of individual's lifeCoordinate and ensure that post-admission paperwork is completed, and that all required consent, records, and other necessary forms are signed as appropriate and filed in individual care records, and updated as per protocolsCommunicate with external referral sources and agencies and obtain and gather all relevant and required records, tests and evaluations, reports, etc. needed for evaluation, risk assessment, review, and service planningDevelop individual Face Sheet, and update these as requiredDocument and maintain contact notes of significant in-person, phone, and other contacts with family members, external workers (case workers and social workers etc.), as well as individual contact, and internal communication, and ensure that these are placed in the individual care record monthlyService Planning: Oversee the development and implementation of Individual Service Plans (ISP) in collaboration with individual's personal choices and preferences, which are individually tailored to his/her strengths, needs and interestsFacilitate and coordinate ISP meetingsCoordinate and collect treatment quarterly reports and treatment goals for ISPWrite and distribute Individual Services Plans, following ISP meetingsCoordinate intake and discharge related planningCommunicate intake and discharge notifications and plans to relevant internal and external partiesCoordinate intake and discharge process, including packing of personal belongings and transportation plans Participate in Individual Services Plan (ISP) meetingsGeneral Responsibilities: Attend team meetings, ISP meetings, in-service trainings, conferences and seminars as required by Director of Adult ServicesAssist in admissions process as assignedCooperate and participate in internal investigationsPerform other reasonably related duty as assignedQualifications: Bachelor's degree in related field of studyMinimum of two years' case management experience in Human Service FieldKnowledge of Department of Developmental Disabilities and Day Habilitation regulations and CARF standards preferredCrystal Springs, Inc.38 Narrows RoadAssonet, MA 02702 (508) ###-#### Springs is an equal opportunity employer, and we are committed to creating a diverse and inclusive workplace.
