Big agency that operates like a small organization with strong connections to the community.Case Manager, Hispanic Outreach, Lake CountyHispanic people have been calling Lake County home for decades, and now a portion of the population is aging. The Hispanic Senior case manager will provide both individualized case management services to assist with health and other related issues, as well as group programming to counter isolation. The case manager may also be responsible for coordinating volunteers. This program will be an additional service offered under our Minority Outreach program and Catholic Charities is already well established in the community. Many of the hours of service may be non-traditional business hours that are likely to include evenings and/or weekends.Responsibilities:
  • Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
  • Provide clients direct service, either in the office or in the field.
  • Review and monitor services, reassess needs, and initiate services which are responsive to the needs of clients; provide service coordination.
  • Assist clients in developing formal and informal community supports.
  • Provide advocacy on behalf of clients.
  • Prepare and complete all required case recordings in an electronic medical record, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
  • Attend clinical and administrative staff meetings, multi discipline team meeting as required.
  • Review treatment plans and progress of cases with supervisor for continued support, advice and approval.
  • Perform other duties as necessary to accomplish objectives.
