Description

Join our HOTT program as a Case Manager and make a meaningful difference in the lives of LGBTQ+ and marginalized youth. You'll provide essential case management and supportive services to young people ages 13-24.

As a HOTT Case Manager, your responsibilities will include:

+ Register new patients and enter their data into our electronic health records.

+ Assess patients' eligibility for our low-cost or no-cost services.

+ Assist patients in enrolling in government-funded health insurance programs.

+ Coordinate referrals to other healthcare providers and community resources.

+ Provide education and support on family planning and various medical issues.

+ Work closely with primary care providers, nurses, behavioral health workers, and outreach staff to ensure comprehensive care for patients.

Work Schedule/Salary:

This is a full-time, 5-day-per-week position including some evening hours. You must be available for early morning, evening, and/or weekend shifts.

Qualifications

+ Bachelors degree in health/social services preferred. High School Diploma or prior experience required.

+ Experience working with special populations of youth (homeless, street-based, and unstably housed)

+ Experience working with LGBT, HIV and transgender health-related issues and communities.

+ Experience in medical/social services (case management) and/or entitlements work preferred.

+ Experience facilitating psycho-education/support groups

+ Strong computer competency, including knowledge of Electronic Medical Records, computerized appointment/billing programs and other Microsoft Office programs.

+ Strong customer service orientation.

+ Bilingual (English/Spanish) strongly preferred.
