We are currently seeking an Intake Coordinator to join our Developmental Disability Service Coordination team. The successful incumbent will be the initial point of contact for individuals seeking developmental disability services by determining eligibility for services and connecting families with resources and community-based support services. This team is essential in helping individuals with developmental disabilities lead more independent and fulfilling lives! Minimum Qualifications: Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field; two years of experience in a human services field; or an equivalent combination of training and experience. Two years of direct service experience with those with intellectual disability/qualified developmental disability diagnosis preferred. Experience with electronic health records preferred. Qualified Developmental Disabilities Professional (QDDP) designation preferred. Required Knowledge, Skills, and Abilities: Working knowledge of case management methods, practices, and procedures. Ability to interview clients to identify deficiencies in personal, social, economic and health needs; to work in a multi-disciplinary team environment providing treatment and rehabilitation services; to communicate effectively orally and in writing; to operate a personal computer and related software and other standard office equipment; to develop and maintain effective working relationships with internal and external customers. Knowledge and understanding of the Developmental Disability (DD) Waiver services preferred; knowledge of various developmental disabilities, their characteristics, and the impact they have on individuals and families preferred; bilingual skills preferred. Additional Requirements: Current valid driver's license and good driving record required. Based on the Virginia DMV point system, record must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or a major violation of six demerit points within the preceding thirty-six months. Out of state driving records must be obtained by applicant and presented at time of interview. Record must reflect at least three years of history and be dated within thirty days of interview. Pre-employment drug testing, FBI criminal background check, and education/degree verification required.Duties Duties include but are not limited to the following: • Conducts comprehensive assessments to determine the individual's eligibility for developmental disability (DD) waiver waitlist and services; • Assists in developing individualized service plans in collaboration with the individual, their family, and other support team members; • Links consumers to other community resources and collaborates with other community agencies as needed; • Coordinates transfers from other community service boards, behavioral health agencies, and related organizations; • Prepares and maintains casework documentation as required in the delivery of services (e.g. case histories, assessments, treatment plans, progress notes, correspondence) • Monitors delivery of services and quality of care; evaluates case plan effectiveness and manages resources; and • Perform other work as required.
