Case ManagerReal-world impact. Life-changing outcomes.CommQuest is currently seeking a Full Time Case Manager to work at our women's residential facility. The Case Manager is responsible for the development and planning of client care and coordination of services and will work in partnership with clinical and other staff members to ensure best client care. The Case Manager will assess, plan, implement, coordinate, monitor and evaluate the options and services necessary to meet the needs of clients. The Case Manager may also be asked to complete jail screens as well.In this role, you will:

Job Duties include:
  • Ongoing assessment of client need(s).
  • Facilitation and development of sober living skills.
  • Effective crisis management.
  • Coordination of the BHSP (Behavioral Health Service Plan).
  • Advocacy and outreach on behalf of clients.
  • Linkage with appropriate resources on behalf of clients.
  • Symptom monitoring and interventions.
  • Willingness to work in a variety of community settings.
  • Client and family education.
  • Facilitate/co-facilitate group counseling sessions. Clinical record keeping in conjunction with facilitation.
  • All other duties as assigned.
Shift:Days: Sunday - Thursday 8:00 AM - 4:00 PM. FTDays: Friday - Sunday 8:00 AM - 4:00 PM. PTMinimum Qualifications:
  • Minimum of a Bachelor's degree in Human Services, Chemical Dependency, or other related field highly desired.
  • Minimum of a current, active CDCA certification through the Ohio CDP Board is required.
  • Previous experience of 1+ year working with adults with Substance Use Disorder and/or mild to moderate mental illness is preferred.
  • Must have the ability to establish working relationships with diverse individuals in a variety of settings and situations.
  • Must possess strong communication skills and the ability to work collaboratively with other providers in the agency and community.
  • Must be willing to submit to pre-employment drug screen and BCI/FBI background check.
