The goal of the Day Center is to build trusting relationships to move women out of homelessness. The Case Manager provides comprehensive supportive services that help participants identify strengths and barriers, and develop action plans that encourages self-sufficiency, self-determination, and that lead to stabilization in permanent housing.This position has a social justice component that requires critical thinking through the lens of racism and intersections with poverty. Knowing the core principals of antiracism and grounding those principles in everyday work are required job skills and core values. As an equal opportunity employer, we highly encourage people of color to apply.ResponsibilitiesComplete screening and intake and assess participants needsAssist participants in developing individualized plans to secure and maintain permanent housing using a strengths-based modelNegotiate goals and objectives that encourage self-sufficiency and self-determination and that lead to stabilization in permanent housing Meet regularly weekly with participants to build trust, provide supportive services and link to other community agencies to resolve barriers to housing and other basic needsProvide housing search assistance Assist in negotiating the social service system, advocate with mainstream service providers when needed, accompany participants to appointments as necessaryEncourage a supportive community environment by coordinating group activities such as community meetings, field trips, and special events Provide housing stabilization assistance (i.e. money management, anger management, and socialization skills)Assist with housing placement including accessing needed move-in services (i.e. deposit assistance, furnishings, etc)Provide support and retention services for at least 6 months after exit from program Work with Program Manager to devise and implement action plan for participants not complying with program guidelines and/or policies. Work effectively with other Angeline's staff Maintain positive relationships and coordinate linkages with other service providers Maintain client records, database requirements, narratives and statistical reportsDevelop and maintain resource filesShow demonstrated ability to interact with people of different cultures and continue search for understanding of racial, gender and class equityAdhere to all Volunteer Services protocol relative to volunteer usage, recognition and monitoring.
