What is this job? The Regional Navigator Case Manager is the member of the care team responsible for initiating and maintaining effective working relationships with youth, families, referral sources and community resources--prior to admission, through placement, and after transition/discharge. This position will support the Regional Navigator Program for youth and young adult survivors of human trafficking. The Regional Navigator Case Manager acts as a single point of contact to ensure effective communication and organized service provision. The Regional Navigator Case Manager is also responsible for the development and review of a comprehensive needs assessment and ongoing, frequent safety planning with the youth. The Regional Navigator Case Manager works to promote and identify resources available for the youth and family in their home community to build natural supports that will enhance their ability to increase self-sufficiency. An essential responsibility of the Regional Navigator Case Manager is family engagement and identification of strengths and needs. The Regional Navigator Case Manager will collaborate with community partners to encourage consistent screening and referrals of suspected survivors of child sex trafficking, advocate for youth needs, identify resources, form working relationships, conduct warm handoffs, and promote public awareness through community education and advocacy. You would join about 800 teammates across DC, MD, PA, and WV that empower youth and their families to flourish within their community. Do I need any prior experience? Required Education and Experience Bachelor's Degree. At least one year of experience in providing direct services to children/youth and families One year of experience in any of the following: Working with survivors of trafficking, sexual assault, domestic or intimate partner violence, or other crimes or trauma. Working in child and young adult systems of care and/or multi-disciplinary teams, such as DSS, CPS, DJS, Child Advocacy Center, law enforcement, or State's Attorney's Office. Working in any emergency or crisis response services, such as mobile crisis, rape crisis or suicide prevention hotlines, EMT, or emergency departments. What does the benefits package look like? Make it happen.” Collaborate with first responders and other community partners to swiftly deliver emergency assistance and essential supplies to designated youth promptly. Complete designated admission service needs assessments that include participation by child/youth, parents/guardians in the identification of strengths, service needs, and transition/discharge planning. Complete ongoing functional assessments at designated timeframes that include participation by child/youth, parents/guardians and appropriate external stakeholders. Participate in regular care coordination meetings, including FTDMs, as well as court appointments, school meetings, community providers meetings, group, individual and family meetings as scheduled. Participate as an active member of a care team for each youth on an assigned caseload. Understanding that the care team includes members who are employed at BCC as well as the youth's family, external health providers, church community, and any support that the youth and family identify. Promote an understanding of available Everstand and community resources. Locate and facilitate referrals to community-based resources within the youth's home community. Coordinate with the Everestand staff members and community resources to address clients' service needs. Ensure that the service needs are meet. Evaluate the child's/youth's/family's progress towards goal attainment, in line with scheduled treatment planning and/or care coordination meetings. Maintain effective working relationships with families, children/youth, colleagues, consultants and community agencies to include scheduling home visits, notifications of serious incidents, follow-up on all ongoing documentation needs. Actively participate in transition and discharge planning for the youth as needed to ensure ongoing support of the youth and smooth transition to identified post-discharge services. Ensure post-discharge follow-up—30 days, 60 days, and 90 days after discharge—to collect data that accurately reflects the residual impact of treatment. Complete required documentation and reports within the defined timelines including case management notes, state data collection, assessments, serious incident reports, treatment plan documentation, transition/discharge planning, post-discharge follow-up. Provide effective case coordination to assigned cases which includes but is not limited to: liaison with and provide referrals and warm handoffs to internal and external professionals such as health and wellness professionals, schools, placing agencies, lawyers, community partners, community-based service providers, law enforcement and other human service agencies. Ensure assigned case records meet documentation standards. Ensure at least once per week face to face or virtual individual contact with each youth assigned to case load and at least once per week contact (can include face to face, virtual, phone or email) with youth's primary external care team members, including family members, unless the youth, family and care team agree that less frequent contact is appropriate due to the youth's stability and progress. Identify supervision and/or professional training needs to supervisor as they arise. Demonstrate the ability to effectively express ideas clearly and concisely, orally and in writing. Demonstrate the ability to engage children and families in a positive therapeutic alliance Collaborate with community partners to encourage consistent screening and referrals of suspected survivors of child sex trafficking, advocate for youth needs, identify resources, form working relationships, conduct warm handoffs, promote public awareness, and complete prevention education in the community. Utilize Everstand's continuum of care to connect youth to appropriate levels of service and consult on cases from programs across the continuum. Actively participate in initiatives and community-based workgroups that promote community outreach, education and collaborative engagement around issues that impact survivors of human trafficking across the communities served by Everstand to ensure the organization is recognized as a valuable community partner. Track and report gaps in services needed or requested by youth. Complete necessary financial and programmatic reporting as may be required by grants and/or alternative funding source requirements. Contact youth within 24 hours following any calls placed to the after-hours emergency line. What shifts would I work? A full-time positions available? This is a full-time position. Days and hours of work are typically Monday through Friday, 8:30 a.m. to 5 p.m. Candidates must be able to work flexible hours – including evening, overnight, weekends and holidays. This position regularly requires long hours and weekend work due to the nature of the population served. Eligible for flex and hybrid remote schedule. This position is non-essential during inclement weather. Other Job Requirements This position requires you to drive Everstand vehicles.
