VETERANS VILLAGE
Golden Isles Veterans Village is a community of thirty tiny homes and a community center designed to help homeless veterans transition to more stable lives. In addition to housing, veterans of the village receive physician and mental health services, education and job training, and have the opportunity to come together as a community to support one another.
JOB SUMMARY
The Case Manager is a permanent position working directly with resident veterans. This leader will establish treatment programs, set schedules and routines, coordinate services, conduct weekly interdisciplinary meetings and evaluations as needed, identify treatment plans, and provide resources as necessary.
MAJOR DUTIES
Develops plans for each veteran that meets specific needs. Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends, compiling statistics that are helpful in reporting. Completes grant and subsidy applications. Studies, evaluates, and re-designs processes to improve treatment results. Maintains client records, logging events and progress. Leads the intake process to determine a veteran's need for therapeutic medical, psycho-social, and psychiatric evaluations, reviewing therapist evaluations and treatment objectives.Communicates progress to the team, identifying what is working and what is not working for individual veterans. Monitors expenses to meet the budget and implements cost-saving actions.Improves knowledge by continuing education and networking in the community. QUALIFICATIONS
Understand cultural backgrounds as stated by the National Association of Social Workers (NASW). Knowledge of human behavioral patterns. Has worked effectively with individual clients and maintains confidentiality.Excellent verbal and written communication skills. Ability to maintain objectivity so that each case is evaluated clearly.Strong observation, interview, and research skills. Understanding of the service landscape and develops strong relationships with resource providers. Ability to provide services with dignity and respect. Knowledge of veterans and their unique needs MINIMUM REQUIREMENTS
1. Bachelor's degree or higher in Social Work or a similar field.
2. Three (3) years or more in the social work field, preferably in human services.
3. Must have a valid in-state driver's license.
4. Demonstrated computer skills.
