Location: South LA & Long Beach – 100% onsite

  • 3 Locations in Long Beach
  • 21 in South LA (from the 405 to the I -10 is the main corridor - Englewood down to long beach)

Schedule: M-F 8a-4:30pm

Type: Contract to Hire

Start: ASAP

Job Overview:

LHH Recruitment Solutions has partnered with a healthcare organization seeking a Case Manager. The primary role of the Case Manager is to provide comprehensive strength-based case management services to clients, including the successful transition into and maintenance of permanent housing. This position is based in South LA/Long Beach and requires on-site presence. This is a contract to hire position with the potential for long-term employment based on performance.

Responsibilities:

  • Monitor and conduct weekly case management meetings with clients on a one-on-one basis making any necessary recommendations for additional services.
  • Wellness/room checks, facilitate groups with participants, viewing housing units.
  • Monitor client files to ensure quality, grant compliance, and all information is accurate and up to date.
  • Maintain client confidentiality in accordance with policy and procedures as well as funding regulations.
  • Aid individuals and/or families struggling with permanent housing, social services, mental health, medical concerns, domestic violence, and parenting assistance (as needed).
  • Locate and make appropriate recommendations for community resources and linkage to services for clients.
  • Evaluate client's readiness to transition; including the formulation of service plans and goals, and aid clients in implementing service plans.
  • Attend internal and external trainings to stay abreast of changes.
  • Attend Multidisciplinary Case Planning and Supervision Meetings.

Qualifications:

  • Bachelor's Degree required.
  • Driver's License required.
  • 1+ year experience working with the homeless required.
  • Looking for: Problem solvers, confidentiality, etc.
  • Ability to handle workload of 20-25 guests per week; 4-5 clients per day.
  • Ability to work 100% onsite in a shelter setting.
  • Ability to handle heavy administrative work- all casework must be entered in statewide database.
