Case Manager/ Support Services Hunt Valley, Cockeysville, MD, USA Req #1245 Monday, December 9, 2024 Case Manager Support Services Rate of Pay $65,000/Annually FLSA Status: Exempt We are seeking a Full-time Case Manager, responsible for the management of Richcroft personal supports, respite, shared living, and supported living that are provided within the Support Services Department. This position is: Monday through Friday 9am- 5pm Job Responsibilities: Supervision of 25-35 individual cases, and direct care staff Provide case management services to the individuals served by Richcroft, Inc. as per policies, procedures, and regulations. Monitor the quality of services via visits and communication with each person/ family receiving support services. Collaborate with external organizations on the development of the annual Person-Centered Plan for each person receiving supports (PCP). Monitor progress of the outcomes from the PCP monthly and complete a progress report every 90 days for all individuals on caseload. Progress notes for all individuals on case load to be completed as outlined in the Individual Plan and submitted to director. Maintain all program files following established guidelines. Provide timely and effective responses to calls. Provide support, training, and day-to-day guidance and direction to staff and individuals of Richcroft, Inc. Keep the Director well informed of any unusual events involving staff, individuals, or the community in a timely manner. Communicate with external organizations as needed. Maintain all required licenses and certifications. Bi-monthly formal supervision with direct support staff. Maintain supervision documentation. Bi-weekly supervision with Director. Bi-weekly payroll review and approval. Weekly monitoring of LTSS for EVV accuracy. Make corrections and edits to LTSS EVV punches as needed. Review and processing of staff reimbursement requests. Participate in staff interviews, hiring, and on-boarding process. Monitor and update staffing needs. Maintenance of Outlook Calendar and working schedule to complete duties as required. Other duties as assigned. May need to fill in for staff who are out. Other duties as assigned. College education is preferred in Human Services, which may include, but not be limited to, Social Work, Psychology, or Education. At least two years of experience in the field of developmental disabilities Must have access to personal use of a vehicle, to conduct agency business, including the transportation of supported individuals.
