- Monday-Friday or Sunday-Thursday; 8:00am- 4:30pm
- Sunday-Thursday or Tuesday-Saturday; 10:00am-6:00pm

Duties and Responsibilities:The Case Manager is responsible for providing field-based direct client services alongside other case managers, therapists, Counselors, and community providers. The Case Manager facilitates the achievement of client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation. Based on the needs and values of the client, and in collaboration with all service providers. The Case Manager links clients with appropriate providers and resources throughout the continuum of health and human services and care settings while ensuring that the care provided is safe, effective, client-centered, timely, efficient, and equitable.

- Utilizes evidence-based practices in service delivery, including intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention, Housing, and aftercare planning.
- Under the general direction of the Supervisor, is responsible for the case management services offered to Clients and their families. Ensures the entire case management process, assessment, evaluation, planning, authorizing, advocacy, monitoring, reviewing, and associated documentation are completed.
- Understands services offered in the community and through the client payor.
- Supports the client with aftercare planning, next steps towards recovery, and outpatient services.
- Performs housing location, Sober living, and transitional living.
- Maintains documentation standards as set forth by the program.
- Maintains compliance with HIPAA policies and practices.
- Completes intake, assessment, and service plans in coordination with the client and within the documents standards set forth by the program.
- Plans for review with participants, and appropriately close cases against specified criteria resulting from program completion or voluntary exit.
- In case of involuntary discharge/termination or other adverse action, work with the program manager to ensure all procedures are followed.
- Supports the client with personal needs (EDD, personal finances, external appointments)
- Regularly reports to supervisor on challenging issues that arise in participants progress, presenting recommendations for service transfers, discontinuances, or extensions.
- Carries a caseload of 10 to 22 clients, liaison with Kaiser and VA clients collaboratively with the current liaisons.
- Ability to work in a fast-paced environment as part of an interdisciplinary team and to establish effective working relations with staff, patients, and community partners.
- Assists with transporting admitted clients to appointments as needed.
- Other administrative duties as assigned.
Required education and experience:
- Must possess a High School diploma or GED.
- Advanced knowledge and experience with substance abuse/ mental health population and case management techniques that include the following: Critical Time Intervention, Harm Reduction Strategies, Crisis Intervention Techniques, and Motivational Interviewing.
- Familiarization with HIPAA/client confidentiality and personal rights.
- Must possess CPR/First Aid certification.
- Proficiency with MS office suite.
- Valid Drivers License and insurable under Sierra Health and Wellness Centers automobile insurance policy.
Preferred education and experience:
- Bachelors degree in human services or related field.
- One (1) year of experience providing intensive case management services to a population with mental health, substance use, or co-occurring disorders.
- Experience with Substance Abuse population.
- Experience working with the veteran population.

Hourly: $25.22 - $32.78 The posted pay range is a reasonable estimate that the organization believes in good faith it may pay for this particular job based on the circumstances at the time of posting.
