Or- A Bachelors degree in social science is required, plus (2) years of experience working in a behavioral health field. Or- Associates degree in social science required, plus (3) years of experience working in a behavioral health field. Valid California Driver's License Meets Agency driver eligibility criteria How you will make a difference (Job Overview): This position coordinates the care and serves as ECM Point of Contact/Lead Care Manager for the Full Circle Health ECM qualifying youth. Division/Program Overview: The ECM services at Capital Star oversee care for eligible TAY youth, acting as the liaison between the Division of Behavioral Health and subcontracted providers. This role involves collaborating with hospital staff on discharge planning, connecting TAY youth to community social services, and coordinating in-person visits with contracted providers. Additionally, the ECM advocates for TAY youth with healthcare professionals monitors treatment adherence (including medication), and employs a whatever it takes approach to providing services across various settings such as homes, schools, and community locations. Responsibilities also include transporting participants, accompanying them to office visits, providing crisis intervention, and facilitating case management and linkage services. All services are documented in the Electronic Health Record, with all the required outcome measures completed. The ECM integrates the Transition to Independence Process (TIP), Trauma-Informed Care (TIC), and Motivational Interviewing into services, ensuring that the FSP treatment team provides comprehensive care in collaboration with the Clinical Supervisor and QA/Training.
