Position Summary: The Case Manager provides case management coordination, individual case planning, monitoring of participants' progress, coordination, and facilitation of progress meetings as well as tracking and reporting individual and program activities. Is accountable for providing comprehensive and excellent services with the goal of achieving optimum outcomes for each participant. Day in the life : In a typical day, a Case Manager can expect to... Individualized Case Management Services: Ensure individuals receive ongoing case planning, individualized and personalized goal setting, feedback on progress toward goals, support and follow up for making progress toward individual goals. Case Management Coordination: Ensure comprehensive and optimal service delivery for all individuals by serving as the main contact and liaison for the individual and cooperating partners, persons and/or agencies. Documentation: Complete and maintain essential documentation about participant needs and the services provided within established timelines and in accordance with applicable procedures, policies and requirements. Ability to meet deadlines and achieve required outcomes Adaptive and open to change and flexibility within job Proven verbal and written communication skills Effective decision making skills Experience successfully managing a case load Knowledge of data privacy requirements and ability to maintain confidentiality of sensitive information Interest and comfort working with individuals and families from diverse cultural, social and economic backgrounds. Ability to work in a team environment. Proficient in Microsoft Office Suite software and the ability to accurately enter data into computer-based systems.
