Role Description

This is a full-time on-site role for a Case Manager at ARIA Kentucky. The Case Manager will be responsible for providing support and guidance to clients. Case Manager will be responsible for assessing and providing for the biopsychosocial needs of clients participating in Outpatient Services. Coordinates with other program staff to develop a multidisciplinary treatment plan to address priority needs. Communicates with participants and their health care providers, following all guidelines set forth by HIPPA, about any addiction, mental health, or physical health concerns that require services. Assures continuity of care between counseling services, other in-house care givers, health care providers, and participants.

Qualifications

• Case Management, Counseling, and Crisis Intervention skills
• Experience working in addiction or mental health services
• Strong communication and interpersonal skills
• Ability to collaborate with a multidisciplinary team
• Empathy, compassion, and patience
• Knowledge of relevant laws and regulations in the healthcare field
• Bachelor's Degree
