Hours : Part-time (Maximum 29 hours per week), schedule to be determined Location : Lynchburg VA, 24501 Compensation dependent on experience Position Summary The Developmental Disability/Intellectual Disability Case Manager serves as a support coordinator, ensuring that services to individuals with developmental disabilities and intellectual disabilities are planned, individualized, and person-centered. The Case Manager provides services that may include assistance to individuals and their family members in accessing needed services that are responsive to the individual's needs. Required Education and Experience Bachelor's degree in from an accredited college/university in an applicable human services field (psychology, social work, counseling, or related field) A minimum of one (1) year of directly related work experience providing case management services to individuals with developmental disabilities. May consider an equivalent combination of education and experience that demonstrates the required knowledge, skills, and abilities. Related Knowledge, Skills and Abilities Knowledge of services, systems, and programs available in the community including primary healthcare, support services, eligibility criteria, intake processes, and generic community resources and treatment programs. Knowledge of the nature of developmental and intellectual disabilities, serious mental illness, substance use disorders, and/or co-occurring disorders, depending on the population served, including clinical and developmental issues. Knowledge of different types of assessments, including functional assessment (i.e., VIDES), and their use in service planning. Knowledge of treatment modalities and intervention techniques, such as behavior management independent living skills training, supportive counseling, community integration, supported employment, family education, crisis intervention, discharge/transition planning, and service coordination. Knowledge of types of developmental, mental health, and substance use services available in the locality. Knowledge of the service planning process and major components of a service plan. Knowledge of the use of medications in the care or treatment of the population served. Knowledge of all applicable federal and state laws and regulations and local ordinances. Skilled in identifying and documenting an individual's need for resources, services, and other supports. Skilled in using information from assessments, evaluations, observation, and interviews to develop individual service plans; needs and documenting how resources, services, and natural supports such as family can be utilized to achieve an individual's personal habilitative/rehabilitative and life goals. Skilled in coordinating the provision of services by public and private providers. Ability to work as team members, maintaining effective inter- and intra-agency working relationships. Ability to work independently, performing duties under general supervision. Ability to engage in and sustain ongoing relationships with individuals receiving services. Ability to complete the DBHDS core competency-based curriculum within 30 days of hire. Ability to participate in and successfully pass behavioral intervention training, which includes both verbal de-escalation skills and physical skills techniques. Ability to drive and must possess a valid Virginia driver's license. May be called upon to travel to any Horizon worksite, transport clients, provide services to clients at other locations. Regular attendance at training may require travel to a variety of Horizon sites. Willing and available to work modified schedules (weekends, holidays, etc.) as required.
