Job Description: ( – New Mexico Mi Via Waiver, Supports Waiver, and Community Benefits)We seek a dedicated and compassionate **Case Manager** to join our team in the Northwest region of New Mexico, including Albuquerque. This role involves supporting individuals participating in the Mi Via Waiver, Supports Waiver, and Community Benefits programs. You will help clients navigate services, access resources, and achieve their personal goals while ensuring compliance with program guidelines.Key Responsibilities:Client Coordination: Develop, implement, and monitor individualized service plans (ISP) tailored to each client's needs and preferences. Assist clients and families in identifying and accessing services, resources, and support. Schedule and conduct regular meetings with clients to review progress and adjust plans as needed.Compliance and Documentation: Maintain accurate, timely, and detailed documentation of all case management activities. Ensure state and federal regulations compliance, including program eligibility and reporting requirements.Advocacy and Support: Act as a liaison between clients, families, service providers, and community organizations. Advocate for clients to ensure they receive appropriate care and services. Provide education on program options, rights, and responsibilities.Collaboration: Build and maintain relationships with community partners and service providers. Participate in team meetings, training, and professional development opportunities.Qualifications:Education: Bachelor's degree in social work, psychology, human services, or a related field. Equivalent experience may be considered.Experience: Previous case management experience preferred, especially in Medicaid waiver programs or similar services. Familiarity with Mi Via Waiver, Supports Waiver, and Community Benefits programs is a plus.Skills: Strong organizational, communication, and interpersonal skills. Proficiency in documentation and electronic health records systems. Ability to work independently and as part of a team.Requirements: Valid driver's license and reliable transportation. Background check clearance.
