Get paid when you want to be paid!Monthly Celebrations.Case Manger/MDS Coordinator Primary ResponsibilitiesThe MDS Coordinator is responsible for leading the MDS process for the facility per the RAI Guidelines. This includes assessing resident's physical and mental functions, documenting data on Minimum Data Set per state and federal guidelines, determining referrals to other health care professionals and using the Care Area Assessments to implement and oversee the resident care plan process.The primary purpose of this position is to communicate care requirements to the interdisciplinary team based onthe clinical needs, coordinate health care benefits for all skilled residents (Medicare, Managed Care,Commercial Insurance, and other alternate payment model beneficiaries).Responsibilities:Case Manager/MDS Coordinator Essential FunctionsOrganizing, managing and completing the MDS process for all residentsAttending morning meetings daily to monitor order changes, significant changes, pay or changes, etc.Complete daily PPS meetings to manage Medicare, Managed Care and Medicaid ProcessesLead or assist in leading resident care plan meetings and following up on issues with appropriate staffMaintaining MDS Schedule and communicating to IDT for PPS and OBRA MDSCompleting all annual, quarterly and significant changes and discharge MDS assessmentsAssist the DON with monitoring the review of nurse's notes to determine if care plans are being followedAttending scheduled in-services, staff meetings, workshop and seminarsComplying with safety requirements, infection control measures, and maintaining a clean work environment in accordance with facility policyPerforming other duties as assignedIND123Qualifications:MDS Coordinator QualificationsMust be a graduate of an accredited school of nursing and possess a valid RN or LPN license in good standing according to State and Federal requirementsMust understand the philosophy of and know the principles of the MDS processMust possess a thorough understanding of the principles of safe, effective nursing practicesMust possess managerial and interpersonal skillsPatience, a cheerful disposition, enthusiasm and willingness to handle residents based on whatever maturity and physical level in which they are currently functioning
