A nonprofit organization in New York City is currently seeking a dedicated and compassionate professional to join one of their growing men's shelters in Brooklyn as a Case Manager.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: 3 to 6 months (possibility of extension)
- Schedule: Monday to Friday (rotating weekends)
- Hours: 8am to 4pm or 9am to 5pm
- Setting: Men's Shelter
Responsibilities:
- Conduct initial intake and assessment of clients' needs and periodic re-assessments
- Prepare initial psychosocial evaluation and update according to regulations
- Obtain and maintain relevant documentation and release forms from clients (i.e., birth certificate, proof of income, medical documents, release of Personal Health Information, etc.)
- Conduct periodic revisions of independent living plans including short-term and long-term client goals
- Provide support, training, and coordination with the other Social Services staff
- Recommend and implement strategies to persuade clients to participate fully in the process in order to transition into permanent housing
- Monitor clients' progress toward their goals via regularly scheduled face-to-face contacts/sessions
- Escort clients to appointments (educational, medical, social service, etc.)
- Assist clients in completing applications for benefits and entitlements and may process applications on clients' behalf
- Follow-up with clients for a period after successful completion of their primary goals to assure client stability
- Perform other duties, as needed
Qualifications:
- Bachelor's Degree
- First Aid / CPR certification
Desired Skills:
- Bilingual
- Certification in Overdose Prevention
- Previous experience in a Shelter setting
