Job DetailsLevel Entry Job Location Green Bay - Green Bay, WI Position Type Part Time Education Level High School Salary Range $17.00 - $17.00 Travel Percentage None Job Shift Varies Job Category Criminal Justice Description The Case Manager/Apartment Monitor provides community-based services to correctional clients following their release and transition back to their home community. This is a part-time position that provides case management and monitoring services to clients referred to Attic Correctional Services. The candidate in this position has extensive responsibility communicating with the referral source and staff to work with the resident to reach the successful completion of program.
  • Perform client pre-admission, intake and assessment procedures, includes orientation to the program
  • Distribution of food staples, linens, bedding, etc.
  • Shop and purchase (using agency credit card) for all client intake food and THS supplies
  • Complete and distribute Incident Report forms as required
  • Process all receipts and send to Admin with allocation sheets
  • Maintain petty cash and monthly reports of same
  • Maintain client files and notes
  • Complete client discharge summary and distribute to Agent
  • Maintain current client status in DOC PDC data system
  • Ensure coordination of all THS services, including maintaining staff schedules
  • Complete monitoring check during scheduled hours
  • Complete log entries, both hard copy and computer, for each monitoring check
  • Communicate with co-workers and coordinate monitoring checks
  • Monitor clients to ensure that they are following program policies, procedures and rules
  • Monitor physical plant to ensure health, safety and security of clients, community and property
  • Respond to emergency issues and alert DOC, law enforcement, co-workers and ACS Supervisor
  • Communicate client issues with assigned agent
  • Ensure that apartments are maintained in a clean and sanitary condition at all times
  • Communicate with landlord/maintenance as needed
  • Make sure to keep lines of communication open with Supervisor and respond to any inquiries made
  • Adheres to all rules and regulations
QualificationsKnowledge, Skills & Abilities:
  • Access to his/her own vehicle.
  • Valid drivers license, good driving and proof of insurance.
  • Ability to travel frequently.
  • Demonstrated understanding and sensitivity to interactions with individuals from diverse backgrounds and cultures.
  • Excellent verbal and written communication skills.
  • Ability to interact with staff, clients, and other parties in an effective manner while maintaining professional boundaries.
  • Strong decision-making abilities and self- motivated.
  • Ability to maintain the utmost confidentiality of all agency information.
Education & Work Experience:
  • High School diploma or equivalent
  • Previous experience working with correctional clients preferred.
