Case Manager (Outpatient) - Social Services - Full Time - Days - Lourdes Health : Job Details

*This is designated as a safety-sensitive position*Salary Range: $18.38 - $25.73Essential Functions

• Collaborate with consumers with severe mental illnesses.
• Designs, implements and coordinates services in order to assist the person in achieving personal goals, maintaining an optimum living environment and to decrease the barriers to recovery.
• Works with consumer to develop relationships (formal and informal) that support recovery and relapse prevention.

Qualifications and requirements

• Bachelors Degree in Social/Behavioral Sciences required. Masters in Social/Behavioral Sciences preferred.
• Knowledge and skills necessary to provide care appropriate to the population of patients served in Case Management.
• Minimum (1) one year of experience working with persons who experience mental illness required.
• Must have/obtain a current Washington State Agency Affiliate, within 60 days of hire date, unless currently a Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) in the State of Washington.
• Must have/obtain a current Washington State Driver's License with dependable transportation. Must have current/valid Liability Insurance to use personal vehicle and/or LHN Company vehicles for LHN business purposes.
• Bilingual and/or English/Spanish-Speaking preferred. Ability to work with culturally diverse population
