Case Manager- Project Link - Alleghany Highlands Community Services : Job Details

Join us as we begin a new program for women who are pregnant, post-partum, have dependent children, or who are of childbearing age and are experiencing substance use or at risk for substance use. Provide intensive supportive services to women whose lives have been impacted by substance use and/or are living with or at risk for mental health and/or substance use disorders. Services include the development and implementation of individualized service plans with individuals and families to promote wellness in the home, school/work, and community. Refer, monitor, coordinate, and advocate for services and needs. Providing education, assessment, and discharge planning services to prenatal and post-partum at-risk women with substance abuse and/or mental illness living in the community or at-risk women who have children under the age of four. The overall goal is to engage parents in needed services and improve outcomes for newborns and families. Education: Bachelor's Degree in Human Services field (required) Experience: At least one year of experience providing direct services to individuals with a diagnosis of mental illness, substance use disorders, or co-occurring diagnosis. Licensure/Certifications: Valid Driver's License (required). Willing to obtain certification as Qualified Mental Health Professional (QMHP). Willing to obtain certification as a Board of Counseling Certified Substance Abuse Counselor (CSAC) and partake in training and supervision. In addition, the Basics of Case Management curriculum through VA DBHDS must be completed successfully within 30 days of employment.
