The Case Manager of Youth Haven provides intensive case management and supervision for a small case load of four to six youth who are receiving shelter services. They partner with youth utilizing a trauma-informed approach to work towards their self-identified goals, reduce barriers to staying housed, and transition to permanent housing.

Program Delivery
Work closely with each youths parent/guardian and/or case worker to coordinate care while the youth is in shelter. Utilize a wrap-around service model to develop and coordinate family meetings towards the achievement of client goals. Attend and contribute to team meetings. Ensure consistent communication and accurate documentation. Lead psycho-educational groups and participate in weekly case staffing meetings. Maintain organized, confidential case files, document behaviors, maintain logs, and complete timely reports. Provide support to employee training and outreach for youth shelter services. Participate in professional development and training. Schedule meetings and update the program team regarding transition planning. Participate in on-call rotation to assist in the referral process and intakes as needed. Case Management Tasks
Provide strength-based case management services and ensure goal setting is client centered. Conduct assessments, coordinate services, develop goal plans, and provide discharge reports. Provide weekly individual case management sessions and facilitate groups with clients. Collaborate with families, social workers, and legal advocates to create housing stabilization plans. Link clients with community resources and provide after-care referrals for unmet needs. Provide after-care reports for up to three months following exit Maintain case files in an organized and confidential manner. Provide support and training to employees as requested. Assist in floor coverage and supervision of youth as scheduled or needed. Attend provider meetings in the community and assist in outreach efforts to help ensure awareness of youth shelter services. Build positive relationships with youth demonstrating empathy and non-judgement. Promote community, ensure and support a safe environment utilizing preventative techniques. Teach healthy communication and daily living skills, motivating youth to achieve their personal goals. Model self-regulation and teach effective conflict resolution skills. Provide transportation for medical, educational and legal needs. Listen to and provide personal attention to individual concerns. Support and provide the need for independent living skills. Supervision and Monitoring
Plan for and supervise tasks that promote healthy living. Manage medications and assist with the achievement of personal goals, homework completion and meal preparation. Train volunteers, interns and relief employees as needed. Report any concerns to Supervisor and Child Protective Services. job requirements
a. Education and Experience
Bachelors degree in a human-service field required. Minimum of 2 years of relevant working experience, including supporting youth, young adults and families experiencing homelessness. Bilingual in English/Spanish preferred. Experience providing case management services with an emphasis on trauma-informed, positive youth development. Experience mediating conflict and crisis with families and across multi-disciplinary teams and service providers. Demonstrated knowledge of MS365 Power Platform. b. Additional Requirements
Must satisfactorily pass criminal history check. Must satisfactorily pass tuberculosis (TB) test. Must have reliable transportation and a valid Washington State driver's license (or be able to obtain one by date of employment). Employees and volunteers who operate their own or Friends of Youth vehicles on Friends of Youth business, must have a safe driving record as defined by Friends of Youth. Employees and volunteers who operate their own vehicle(s) on Friends of Youth business must carry auto liability insurance that complies with the requirements of Friends of Youth. Must successfully complete the following health certification requirements within 30 days of employment: CPR, First Aid and Bloodborne Pathogens. Proof of COVID-19 vaccination is required.
