TheCase Manager provides and supports the wellness and recovery goals of individuals with HIV/AIDS and/or chronic physical and mental health needs. He/she is responsible for implementing targeted interventions designed to provide timely, high-quality, and effective care. Case Management services are tailored around goals aimed at providing services that encourage individuals to improve wellness. The Casa Manager will demonstrate skills and competencies required for the position including communication, cultural competence, training professional experiences and education. The activities are tailored to meet the unique needs of the communities.
The salary range is $60-65k annually
KEY ESSENTIAL FUNCTIONS:
+ Maintains a caseload and meets with patients with regarding optimum health or improved functional capability in the right setting.
+ Plans and develops treatment plans and performance goals while monitoring follow up on patient care in order to meet an individual's needs.
+ Ensure that all eligible funding sources are tapped before accessing Ryan White funds.
+ Abide by current RW Standards of Care, including RW eligibility documentation.
+ Ensure community follow-up to engage the client in care; promote compliance with medical appointments and encourages client self-sufficiency and empowerment.
+ Creates more effective linkages between vulnerable populations and health care system.
+ Provides culturally appropriate health education on topics related to HIV/AIDS and chronic disease prevention, physical activity and nutrition, and cultural competence.
+ Provide HIV risk reduction education and prevention.
+ Manages care and care transitions for vulnerable populations.
+ Advocates for undeserved individuals to receive appropriate services.
+ Provides informal counseling.
+ Builds community capacity to address health issues.
+ Provide care management services to patients living with HIV/AIDS and their families/support systems through ensuring access to care, engagement in care coordination of care to obtain the full range of needed services.
+ Gather enrollment consents, RHIO consents, and complete screening, baseline-risk assessments, reassessments, plans of care, plans of care updates and notes in accordance with HRSA and departmental policies.
+ Demonstrate the ability to clearly articulate, verbally and in writing the aims and goals of the HRSA Ryan White Part C program to potential patients, community members and staff.
+ Participate in quality improvement activities, projects and reviews.
+ Complete daily, weekly, monthly or other periodic requests for narrative or quantitative data reports for program review.
+ Enter units of service and client data on a timely basis into NextGen and AIRS Systems.
+ Conducts monthly chart reviews and compiles monthly reports.
+ Identify new individuals with HIV to provide case management services and link to care.
+ Participates in care conferencing.
+ Meet regularly with supervisor and attend staff meetings and case conferences, discusses case management and operational issues impacting performance and program operations.
+ Maintain and update caseload, stratify patients into categories (low, medium, high risk) Provide and review list with Supervisor on a weekly basis.
+ Ensure patient is attending scheduled medical and social service visits through building relationships with patients and providers.
+ Coordinate and schedule appointments with various internal or external providers for appropriate care.
+ At a minimum, maintain once a month contact with each client or at a greater frequency as indicated by the risk satisfaction and plan of care.
+ Access and respond per agency guidelines to client complaints of grievances.
+ Conduct outreach and engagement in accordance with HRSA's policies via phone, electronic methods and letter and or field work to client/collateral/provider to engage clients or strengthen connectivity.
+ Help maintain health and wellness and prevent secondary disease complications through provision of health information, support plan of care and coaching.
+ Promote and expanded linkage development in all areas related to patient care including social determinants (e.g. housing, employment) and monitor the effectiveness of linkages with other service providers via phone, face to face meetings of formal case conferences.
+ Communicate effectively with Supervisor in identifying strengths, weakness and opportunities of program operations.
+ Identify and attend training for professional development and attend departmental in-service meetings as required.
+ Performs other duties as assigned.
REQUIREMENTS:
+ HS Diploma or GED required. Associates or Bachelor's degree preferred.
+ HIV/AIDS Casemanagement experience preferred.
+ Six years case management experience with HS Diploma/GED or four years case management experience with Associates or two years experience with Bachelor's.
+ Must have knowledge of community resources and counseling/social work practices with high risk populations.
+ Experience working with persons in crisis while attaining the ability to work independently with a strong sense of focus.
+ Must be tasked-oriented with ability to be non-judgmental and while maintaining boundaries.
+ Must be able to work in a variety of settings with culturally diverse families while having the ability to be culturally sensitive and appropriate.
+ Moderate level of electronic health record/electronic medical records systems.
+ Knowledge of standard Microsoft Office suite applications and internet.
+ Current Basic Life Support Certification.
+ Sound understanding of all federal, state and oversight agency regulations.
+ Knowledge of disease prevention.
+ Ability to work on a strong team of professionals in a culturally diverse environment.
+ Ability to multitask and to work and function under pressure.
+ Bilingual - Spanish strongly preferred
