Job DetailsJob LocationCastle Gardens (West Harlem) - New York, NYSalary Range$50,000.00 - $55,000.00 Salary/yearTravel PercentageNegligibleJob ShiftDay ShiftDescriptionTitle: Case ManagerUnit: Scattered Site HousingReports to: Program DirectorStatus: Full Time, Regular, Non ExemptSalary: $27.47 to $30.22 (approximately $50,000 to $55,000 annually)Location: 625 West 140th StreetDays/Hours: Monday - Friday 9am -5pm (Some flexibility required)Position Summary:The Case Manager will perform initial assessments, provide ongoing supportive services both in the field and in the office and maintain documentation requirements for justice involved clients with histories of mental illness, substance use and homelessness. Essential Duties and Responsibilities:Conduct intake needs assessments on clients eligible and accepted for housing services;Create individualized service plans and Outcome Stars for each client. Document all client services, including successful and unsuccessful outreach attempts;Conduct crisis intervention with clients, as needed;Assess client's need for temporary rental subsidy and furniture/moving allowance;Assist clients in obtaining all necessary documentation to facilitate benefits application process. Assist clients with completion of and submitting client's application to appropriate benefits provider agency (e.g., HRA, Social Security);Act as a liaison to representatives of benefits administrations;Act as liaison in conjunction with property management, between client and landlord, to mediate any landlord-tenant issues andProvide ongoing counseling, supportive services needed to achieve goals stated in client service plan and Outcome Star.Travel across the 5 boros on mass transportation as assigned and as needed to provide services to assigned participants.QualificationsQualifications:Education- Associate's Degree in Human Services or/and five years of experience working with special population; a High School Diploma and eight years of experience working with special population or a bachelor's degree in human services is preferred with at least 2 years of experience with the target population. CASAC or CASAC-T certification with 2 years' experience working with population is preferred.Work Experience- Candidate must have the ability to oversee a maximum case load of 15 participants.In addition, must be able to provide a collaborative assessment of needs, create and implement service plan, monitor progress and evaluate services provided to the individual participant.Computer Skills- Proficient in Microsoft Excel, Word, and Outlook with a demonstrated experience in accounting software and working knowledge of PowerPoint.Communication Skills -Candidate will possess the ability to have excellent writing, editing and verbal communication skills.General Work Behaviors/Skills - Excellent interpersonal skills with ability to interact culturally, linguistically, and diplomatically with diverse internal and external individuals.The Fortune Society encourages individuals to apply who have successfully made the transition from incarceration and/or substance abuse into a stable, productive lifestyle in the community.Travel Requirements- Physical ability to travel throughout the 5 boroughs via public transportation in accordance with participant location(s).Physical Demands - To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
