We are seeking a dynamic, hard-working, and creative who shares our passion for providing exceptional services to children and families.Under the supervision of the Department Director of Social Services, the Case Manager provides case management service to support families seeking support. Provide case management services in order to identify participant's needs and service preferences and work with participant (and family when appropriate) to develop, implement and oversee a care plan to meet mutually agreed upon goals.Your responsibilities will include:Perform intakes, referrals, and assessment with utmost care and compassion. Provide case management services to clients, including assistance with obtaining/recertifying for public benefits; establishing/maintaining links to primary and behavioral health care; supporting activities and daily living skills; utilizing community services; shopping; etc.Provide services and information in a culturally sensitive and trauma-informed mannerTimely completion of required documentation for participants and data management.Provide crises intervention, as necessary, utilizing de-escalation techniques.Collaborate with other internal departments and outside agencies.Refer participants to community programs, medical services, immigration Managing a high risk and crisis driven caseload, with constant regard to limited time constraints.Conducting emergency assessments and program intakes, providing appropriate urgent referrals for eligible clients, and providing immediate advocacy and assistance with obtaining public and private benefits to sustain stability.Coordinate with the Department Director of Social Services case conferences, case consultations, and case record reviews to ensure a high standard of service delivery and program compliance with all regulatory guidelines regarding participant servicesAssisting with identification of service delivery gaps and developing service linkagesSupporting and mentoring Case Managers/interns by providing leadership and peer coaching and co-facilitating training with supervisory team members.Coordinating and facilitating groups in areas including but not limited to life skills. Attend staff meetings, supervision and training as requiredTask may be modified, expanded and/or assigned over a period of timeQualificationsEducation and Experience:Bachelor's degree in behavioral sciences, human services or related fields with a minimum of one (1) year of professional experience providing case management services for vulnerable populations.Bilingual Spanish/English (Speak, Write, Read)Ability to present a positive demeanor to individuals, their families, staff supportive services personnel, and publicProficient presentation skills as well as oral and written communication skills. Proficient in utilizing Microsoft Excel, PowerPoint, Word, and OutlookExcellent time management and organizational skills Flexible; team player; open to feedback; creative Supervisory ResponsibilitiesThis position has no supervisory responsibilities. Work Environment The position is in an office environment. The dress code is business casual. This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.Travel Travel is expected for this position. Travel throughout the different sites is expected for this position (on a need basis). Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. This is generally a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary.Work Schedule This is a full-time position.
