Job DetailsJob Location: Pikeville - 173 Redale - Pikeville, KYPosition Type: Full TimeEducation Level: High SchoolSalary Range: $19.50 - $19.50 HourlyJob Shift: DayDescriptionPosition Summary:The Case Manager collaborates with WestCare staff and external bodies to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below.Essential Job Functions:Provides case management services and assistance to program participants struggling with substance abuse, and those in recovery;Assesses the needs, conditions, and desires of assigned program participants;Educates and refers program participants to community agencies and businesses that appropriately address their needs for food, shelter, clothing, employment, transportation, etc.Assists participants with establishing or re-establishing community support systems;Acts as advocate on behalf of program participants to improve access to vital services;Participates in clinical staffing decisions regarding the participant's current strengths and resources, and their need for additional resources;Completes documentation of services provided in a timely manner;Provides input into the patient's treatment needs and disposition;Conducts assessments of program participants and provides therapy as needed;Reviews and documents all clinical services and activities;Participates in case conferences involving assigned program participants;Prepares Discharge Summaries, as needed;Assists other WestCare staff as needed to promote cooperation and portray a professional image to the community;Embraces and embodies the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation;Performs any other duties as assigned.QualificationsEssential Qualifications:Certifications/Licensures: Veterans are preferred; CADC or similar certification preferred; Preference will be given to certified candidates; Valid driver's license and the ability to obtain and maintain WestCare's vehicle insurance is required.Education: Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology or other field related to the position is preferred; High School Diploma or equivalent is required.Experience and Competencies: Experience working with individuals suffering from mental health and substance abuse disorders required; At least two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred; Ability to define problems, collect data, establish facts, and draw valid conclusions required; Excellent communication skills are required; Ability to exercise good judgment and discretion required; Ability to work well in a team environment required; Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality required; Professional appearance and demeanor required; Must be culturally/linguistically sensitive to populations served; Intake and assessment experience is highly preferred; Bilingual ability is preferred; Prior experience using office equipment, including phones, scanners, printers, and fax machines, and web cameras is highly desired.#J-18808-Ljbffr
