Position Overview: Under basic supervision, provides case management services for HIV/STD clients of the Public Health District (PHD) clinic; performs intake assessment and develops plan of care. Minimum Qualifications: Required : Bachelors Degree in Social Work, Mental Health, Health & Human Services or a related field 1 year of experience in HIV Case Management, Mental Health Social Services or Public Health Programs OR an equivalent combination of education and experience. Preferred: Licensed Baccalaureate Social Worker from Texas State Board of Social Worker Examiners Essential Functions: Completes initial intake and assessment of clinic clients and develops plan of care; provides ongoing case management services and refers clients to other social service and health care agencies; maintains client records and database; evaluates client progress and lab tests and provides counseling services.
