With the supervision of a social worker, providing timely and complete assessment of social factors that impact on all patients assigned and planning appropriate interventions.

Providing concrete supportive services to patients and families.

Providing information about community resources and appropriate aftercare referrals.

Escorting patients into the community and making home visits as needed.

Collaborating with the multidisciplinary team.

Working closely with related social and community agencies.

Participating in interdisciplinary Performance Improvement Projects.

Required to do electronic documentation.

Other tasks that may be assigned.

Minimum Qualifications1. A Baccalaureate Degree from an accredited college. 2. Section 424-a of the New York Social Services Law requires an authorized agency to inquire whether a candidate for employment with child-caring responsibilities is or has been the subject of a child abuse and maltreatment report.
