Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Duties & ResponsibilitiesEnglish-Spanish bilingual skills a mustCaseworkers, under supervision, in collaboration with multidisciplinary teams, identify psychosocial issues and concerns of patients and their families and assist in providing appropriate services to meet their needs.Tasks may include:
  • With the supervision of a social worker, providing timely and complete assessment of social factors that impact on all patients assigned and planning appropriate interventions.
  • Providing concrete supportive services to patients and families.
  • Providing information about community resources and appropriate aftercare referrals.
  • Escorting patients into the community and making home visits as needed.
  • Collaborating with the multidisciplinary team.
  • Working closely with related social and community agencies.
  • Participating in interdisciplinary Performance Improvement Projects.
  • Required to do electronic documentation.
  • Other tasks that may be assigned.
  • Minimum Qualifications1. A Baccalaureate Degree from an accredited college. 2. Section 424-a of the New York Social Services Law requires an authorized agency to inquire whether a candidate for employment with child-caring responsibilities is or has been the subject of a child abuse and maltreatment report.
