The Central Intake Nurse LPN facilitates referrals, coordinates skilled and non-skilled services, and other community resources through collection and input of data, while demonstrating positive interrelationships and service excellence in the performance of duties by meeting or exceeding the expectations of internal and external service groups.Location:Home Care-Erie CountyRecruiter:Adrian MuddMinimum or Preferred Qualifications:HS - High School Diploma or GED required. 1 year of experience in Medical/Surgical Nursing required. 1 year of experience in a Home Care setting preferred. Licensed Practical Nurse upon hire required.
