Let's do this. Let's change lives one patient at a time. Bellin Health is currently seeking the unsung heroes of the health care profession, Licensed Practical Nurses (LPNs). As an LPN, you will play a crucial role in providing exceptional care to our patients in a primary or specialty care clinic. You will be responsible for gathering patient information, such as medical history, health concerns, and symptoms. You will collaborate with doctors and other healthcare professionals to assess the patient's condition and deliver treatment. In addition, you will administer medication and vaccinations, communicate various treatment plans, along with monitor and maintain any medical equipment.
On a given day as a clinic LPN/MA, you will play the role of a collaborator, educator, and advocate for the patients. You will spend time: - Accurately recording patients' medical history, vital statistics and test results into EPIC
- Conduct screening, diagnostic treatment and rehabilitative, and other supportive services which is part of a team based care model approach in our clinics
- Identify patient problems and assists in care planning as integral part of the treatment team
- Prepare and administer medications and immunizations under guidance/orders of RN or provider
- Provide patient education on healthy living
- Manage patient flow in the clinic
- Maintain exam rooms and equipment to be within infection control standards
- Obtain specimens as provided by provider
- Perform follow up calls to patients
