Job Summary Provides In-Home personal care: hygiene, oral care, hair care and other cares for consumers. Also provides household chore services, cleaning, vacuuming, etc for consumers as established by Long Term Social Services. Reports any changes observed in condition or behavior of patient or unusual incidents outside of established parameters. Collaborates with nurse and supervisor in supporting work assignment. Completes and submits documentation requirements, reports and records visits via Electronic Visit Verification. Performs other duties as assigned. Maintains confidentiality of consumer, visitors, family, personnel and organization business in general. Education and/or Experience * Education: Must be > 16 years of age. High school diploma or equivalent work experience preferred, including ability to read, write and simple math. Basic Life Support Certification is mandatory within six months of employment. Must comply with 6 hours of annual training. * Experience: Must be C.N.A. (Certified Nursing Assistant) certified within four months of employment. Understanding or experience in health care helpful. * Background Check is required. Must have valid driver's license. Licensure/Certification/Registration * C.N.A. certification is required. Pay The hourly minimum wage for this position is $16.50. Actual hourly rate dependent upon experience.
