POSITION SUMMARY:The Hospice CNA is a certified paraprofessional who works under the supervision of a Registered Nurse or Therapist. The CNA also performs various personal care services as necessary to meet the patient's needs. The CNA is responsible for observing patients, reporting these observations and documenting observations and care performed while performing patient care.POSITION EDUCATION/QUALIFICATIONS:¨ High school diploma/GED preferred. ¨ Current Texas State CNA License required. ¨ Current BLS required ¨ Valid and current Driver's License required ¨ Valid and current proof of Auto Insurance required. ¨ Computer skills required with knowledge of Microsoft Office suite. ¨ Good written and verbal communication skills required. ¨ Some evening or weekend work will be required¨ Bilingual - English/Spanish.JOB KNOWLEDGE/EXPERIENCE:¨ Two (2) years Hospital experience preferred.¨ Requires excellent interpersonal skills with families in crisis.¨ Knowledge of State, Federal, and JCAHO regulations.POSITION RESPONSIBILITES: ¨ Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices.¨ Provide and document personal care services such as bed, bath, skin care, shampoo, hair set and comb out, caring for the teeth or dentures, helping patient in and out of bed, bed making, nail and skin care, and toileting and elimination, as required by the patient's plan of care.¨ Assist patient with transfers, walking and ROM exercises, as required by the patient's plan of care.¨ Assist patient with feeding as requested.¨ Assist in delivering of patient supplies as needed.¨ Support the skilled plan of care by carrying out and recording specific nursing care procedures such as applying heat and cold, emptying catheter and colostomy drainage bags, and providing emotional support and encouragement to terminally ill patients and family members.¨ Document observations and services provided daily. Report observations and activities to the registered nurse/case manager responsible for the patient. Notify the registered nurse/case manager immediately of any change in client's condition which may need agency and/or professional intervention.¨ Participate in in-service education programs, staff meetings, case conference, IDT meetings and other opportunities for learning provided.¨ Carry out work according to agency policies and procedures and in such a manner as to reflect appropriate agency standards for timeliness, appearance, and behavior. Maintain confidentiality of client and agency information.¨ Share in providing 24 hour, seven day week coverage to Hospice patients/families.¨ Participate in a minimum of 12 hours of in-services annually.¨ Other duties as assigned.
