General Summary:
* Assesses and identifies the spiritual and emotional needs of hospice patients and families as well as staff and volunteers in accordance with the hospice concept of care.
* Coordinates with the Interdisciplinary Team to provide patient/family care.
* Responsible for the implementation and management of the hospice bereavement program.
* Maintains continuity of care by ensuring patient needs is met.
EDUCATION REQUIRED:
* College graduate with a background in pastoral studies or equivalent studies.
EXPERIENCE REQUIRED:
* Past experience in dealing with terminal illness and bereavement issues is helpful.
* On-the-job Training: A 4-8-week orientation period is recommended to familiarize the employee with policies and procedures as they relate to the hospice program.
