C Train - Van Siclen Avenue

SCOPE OF ROLE: Provides direct services to Supportive Crisis Stabilization Center 24 hr program.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Provide strength based, Person centered, trauma-informed, Social determinants of health assessments of childrens biological, educational, psychological and social needs.
  • Engage is collateral visits with health professionals, school personnel, and is able to make necessary referrals for mental health, daycare, and summer camps.
  • Coordinate activities designed to meet the individual needs of children (e.g., infant stimulation, toddler training, teen programs, after school programming, parent/child interactions, etc.)
  • Attends staff meetings, in-serving training, case conferences, and weekly clinical meetings.
  • Provide weekly and monthly reports
  • Provide crisis intervention to children and parents.
  • Support families in their child care efforts, both in structured and unstructured
  • Complete all progress notes, treatment planning, written assessments, and other required documentation.
  • Help the child/ family get ready for tests or procedures (Medical/Mental Health).
  • Help the child develop coping skills and manage distractions.
  • Provide behavioral Interventions (e.g., reward good behavior; prevent negative behavior).
  • Provide family support; support for brothers and sisters.
  • Provide Play that fits your childs level of understanding.
  • Therapeutic play (e.g., role-playing and storytelling)
  • Provide support for grief and loss. Additional duties as assigned
Qualifications:

REQUIRED EDUCATION AND EXPERIENCE

  • Masters degree and LMSW/LMFT required
  • Experience working with people diagnosed with mental illness and people struggling with substance use and co-Occurring disorder.
  • Experience working within a respite/residential or crisis 24 hr program
  • Effective written and oral communication skills.
  • Computer literacy skills (Excel, Teams, Windows, Outlook)
  • Experience working with children/youth and /or Families
  • Ability to work evenings and/ or weekends
  • Knowledge of substance use and mental illness as it relates to working with families, Youth/children and adults.
  • Effective interpersonal and written communication skills and ability to work as a member of an interdisciplinary team; able to relate with diverse staff; customer service focus in interactions with diverse client population.
  • Ability to document professional clinical notes within appropriate timelines and input in EHR system. Bilingual English/Spanish speaking preferred
