POSITION SUMMARYProvides care and services for children and families with multifaceted needs that may include stabilization in living environment, coordination of placements, and support and services when discharging from higher levels of care. Services provided are primarily in the community and client's living environment. Services may include comprehensive assessment and treatment planning, individual and family therapeutic interventions, close collaboration with clinical team to determine and guide service provision. Position will lead a team to navigate and meet the comprehensive needs of the child and family.ESSENTIAL RESPONSIBILITIESProvide regular and consistent services and supports to clients identified appropriate for community supportsConduct comprehensive assessments of children (ages 6-18) to determine mental and behavioral health needsProvide individual and family therapeutic interventions as appropriateManagement of clinical treatment including crisis assessment and support as neededWork closely with other members of the Community Care Team to insure client's integrated and holistic care needs are metService provision shall be conducted primarily in the client's living and other environments as necessary or appropriateProvide outreach and engagement when neededClosely collaborate with other care teams within Polara to coordinate and meet client needs (Inpatient Hospital, Crisis Stabilization Unit, Outpatient Services, Primary Care)Attend Child and Family Team Meetings (CFTs)Participates as a team member and attends scheduled staffings and supervisionPartner with client, family, and team to identify treatment goalsMaintains clinical record per agency policy and procedureAttends all team meetings, case reviews, staff meetings, supervisory sessions, and in-service trainingsManagement of all aspects of treatment, including referrals to additional resources when appropriatePerforms other duties as assignedQUALIFICATIONS21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice; or minimum 18 years old for select administrative positionsMust be able to provide or obtain an AZDPS Level One Fingerprint Clearance CardMust possess and maintain a valid Driver License, current automobile insurance, and a driving record that meets the guidelines and requirements of the organizationMaster's degree in Counseling, Psychology, Social Work, or a related field.Current licensure or eligibility for licensure as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or similar license in the state of Arizona.Experience working with clients in a mental health setting is preferred.Strong knowledge of evidence-based therapeutic approaches for treating mental health disorders.Ability to establish rapport and build therapeutic alliances with diverse populations.Excellent communication and interpersonal skills.Empathy, compassion, and a non-judgmental approach to client care.Familiarity with electronic health records (EHR) systems and basic computer skills.Commitment to maintaining the highest ethical and professional standards.CORE COMPETENCIESPeople - Passionately believes in people and embraces diversityIntegrity - Does the right thing even when no one is lookingLearning - Challenges old paradigms with critical thinking, curiosity and creativityAccountability - Sets an intentional culture of personal responsibility; believes everyone carries the loadRelationships - Serves colleagues and consumers with love, empathy and compassionPHYSICAL DEMANDS & WORKING CONDITIONSThe physical and working demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
