Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Minimum Qualifications• Graduation from an accredited nursing program• Licensed as Registered Nurse and licensed to practice in North Carolina/South Carolina• Experience in Case Management preferred.• Knowledge of Word and Excel• Experience with EPIC platform for electronic health record.Knowledge, Skills and Abilities• Ability to define problems, collect data, establish facts, and draw valid conclusions.• Ability to interpret and implement a variety of technical instructions and deal with multiple abstract concepts.• Adaptable to variety of situations, some difficult in establishing patient rapport.• Ability to apply information from EMR to develop nursing care plans• Knowledge of treatments, medications, and preventative health care measures in patients with chronic kidney disease• Problem solving skills.• Knowledge of Community Resources to assist patients with Social Determinants of Health• Ability to organize tasks, work as a team or independently.• Social and Cultural Sensitivity and appropriate to ethnicity and economically diverse patient and employee base• Communicates through appropriate channels and proper use of chain of command.• Ability to handle emergency situations calmly and effectively.• Must be able to navigate electronic health records, word, excel, and CCM platform and documentation tool when selected.• Good customer service skills• Be courteous and respectful to patients, care givers, staff, and providers.• Maintain HIPPA and adhere to organizations policy and procedures.Supervisory Responsibilities: NoneEssential Functions
- Manages overall CCM Program - 20%
- Manages a case load of patients that qualify for Chronic Care Management. Manages caseload of assigned patients - 70%
- Assists with development of processes that support Eastern Nephrology Associates patient centered care programs - 5%
