The Resource Center has been providing services to persons with disabilities in the Chautauqua County area since 1958. From our humble beginnings as a gathering of parents and concerned citizens who wanted to provide educational and training opportunities for persons with developmental and intellectual disabilities, The Resource Center has grown into a comprehensive agency providing services to thousands of persons with all types and levels of disabilities. The Resource Center is the largest non-governmental employer in Chautauqua County with more than 1,300 employees on its payroll. The mission of The Resource Center is to support individuals with disabilities and other challenges in achieving maximum independence, contributing to their community, experiencing lifelong growth, and enjoying quality of life. TITLE: Nurse Practitioner/Physician Assistant RESPONSIBLE TO: Executive Director FUNCTION: To ensure the consistent provision of high-quality primary medical care, emphasizing health promotion and disease prevention, as well as management of acute and chronic diseases at Community Inclusion's clinical sites at TRC Community Health Center and/or its School Based Health Centers in the Jamestown City School District. The NP/PA will primarily be based out of one outpatient clinic and may cover at times in other clinic sites. NP/PA will be responsible to coordinate the provision of healthcare between Primary Care/SBHC and other services and other medical providers. SPECIFIC DUTIES: Primary Care Clinical Setting:

  • Adhere to Primary Care protocols and procedures in the compliance with New York State Health Department regulatory standards with Nurse Practitioner (NP) or Physician Assistant (PA) Scope of Practice. Practice Information for Nurse Practitioners | Office of the Professions (nysed.gov)
    • Protocols and procedures may include but are not limited to:
      • ordering diagnostic and laboratory tests
      • prescription of pharmacologic agents and treatment and non-pharmacologic therapies.
      • development of the plan of care and treatment/follow-up as required.
      • communicating test results to patients and their families
      • recording patient medical histories
  • Provide comprehensive patient care and establish a family partnership whenever possible to ensure the needs of the patients are met on an ongoing basis.
  • Ensure high quality primary care is delivered cost-effectively, consistently
  • Provide appropriate specialist referrals and follow-up recommendations when indicated
  • Provide patient education regarding specific health issues. Teaching and counseling individuals, families and groups as a major part of the practitioner's role.
  • Maintain documentation of the patient's medical appointment to Community Inclusion's standards with high efficiency.
  • Make referrals to other medical practitioners/specialists, laboratories, hospitals for further medical evaluation as the need arises.
  • Engage the patient, family and cross-functional care teams in sustained health and well-being of the patient.
  • Support performance improvement initiatives with timely and accurate data and consistent actions.
  • Consults on a regular basis with Primary Care physician/collaborative physician regarding specific patient issues.
  • Participates with primary care physician/collaborative physician in the review of primary care patient files to evaluate and review the type and quality of patient care.
  • Monitors and implements infection control practices and ensures that all practices with the primary care unit are in compliance with agency, State and Federal guidelines.
  • Provide professional supervision and advisement to primary care nursing staff including instruction in performance of job duties.
  • In collaboration with management and practice guidelines, maintains a schedule of appointments to meet the daily expectations of the practice and needs of the patients.
  • Maintains general medical office practices including medical record maintenance and billing practices.
  • Monitor units of service, productivity and expenses to align with department budget expectation.
  • Is knowledgeable and can implement current first aid and/or emergency procedures.
  • Participate on committees as required to provide professional guidance and advisement as necessary. Acts as health care resource, interdisciplinary consultant and patient advocate.
  • May provide in-service education on medical health care issues as needed.
  • Provides excellent quality care and customer service.
  • Responsible to respond to on-call services outside of business hours as assigned by Practice Manager.
  • Performs all other duties as requested by the Physician and Practice Manager.
  • School Based Health Center Setting: Students perform better when they are at school and ready to learn. Working cooperatively as an integral support within the school setting, in addition to the above, a NP/PA in this setting can expect to:
  • Provide a comprehensive range of services that meet the physical and behavioral needs of children and adolescents in middle and/or high-school.
  • Services may include but are not limited to:
    • Age appropriate well-child exams
    • Immunizations in accordance of NYS DOH School-Age Immunization Requirements
    • Diagnoses and treatment of acute illness and injury
    • Management & monitoring of chronic conditions
    • Basic laboratory services,
    • Capability to prescribe commonly used medications
    • Basic mental health services.
  • Support the community - solicit community input for unmet health needs & trends
  • Establish collaborative relationships with patient's primary care physician and/or specialists & school supports.
  • Advance Health Promotion Activities through classroom and/or school-wide health promotion activities.
  • REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Ability to work with adults and children, knowledge and abilities commensurate with those of general medical practice. Awareness of Primary Care Office Management and procedure, knowledge and certification in First Aid and/or emergency procedure, the ability to communicate verbally and in writing. Physical condition commensurate with job responsibilities. Ability to work both autonomously as well as collaboratively with a variety of individuals to diagnose and manage patient's health care problems. Written and verbal communication, listening, organization, time management and teamwork skills, must have Empathy and compassion. MINIMUM QUALIFICATIONS: New York State Certification as Nurse Practitioner** or Physician Assistant Must be eligible to be credentialed and enrolled with insurances and maintain such as required **Collaborative Agreement if fewer than 3,600 hours of practice - see NYS Office of Professions site for current details ** Note: Please review current NYS Nurse Practitioner Practice Act for requirements for Physician Collaborative Agreement requirements, as this is an evolving legislative topic This is a position that is exempt from overtime provisions of the Fair Labor Standards Act. As an Exempt employee, incumbent will receive an annual salary commensurate with assigned responsibilities and salary classification; however, incumbent is expected and required to work as many hours as necessary to fulfill job responsibilities.
