Summary:
The Client Care Coordinator is responsible for managing and overseeing client care services within the JanCare home care agency, ensuring a high-quality and personalized experience for all clients. This role includes managingconcierge services, following up with clients and conducting quality assurance and supervisory visits. The Client Care Coordinator will play a critical role in maintaining client satisfaction, ensuring compliance with agency standards, and enhancing the overall client experience.
Key Responsibilities
Client Care Management:
Coordinate and manage the daily operations of client care services, ensuring all client needs are met promptly and effectively.
Conduct initial assessments and develop personalized care plans in collaboration with clients and their families.
Maintain regular contact with clients and their families to monitor satisfaction and address any concerns or changes in needs.
Ensure all care plans are up-to-date and accurately reflect the client's current status and preferences.
Concierge Services Management:
Oversee the provision of concierge services, including personalized assistance with scheduling appointments, transportation, and other non-medical support services.
Develop and maintain relationships with local service providers to ensure high-quality and reliable concierge services for clients.
Ensure all concierge services are delivered in a timely, professional, and courteous manner.
Quality Assurance and Supervisory Visits:
Conduct regular quality assurance visits to clients' homes to assess the quality of care being provided and ensure compliance with agency standards.
Perform supervisory visits to monitor caregiver performance, provide feedback, and offer support and training as needed.
Document and report findings from quality assurance and supervisory visits, and follow up on any identified issues to ensure prompt resolution.
Monitoring Rehospitalization Data:
Monitor and track rehospitalization data to identify trends and areas for improvement.
Analyze rehospitalization data to develop strategies for reducing hospital readmissions and improving client outcomes.
Work with caregivers and clinical staff to implement interventions aimed at reducing rehospitalization rates.
Communication and Collaboration:
Serve as the primary point of contact for clients and their families, providing clear and consistent communication regarding care plans and services.
Collaborate with caregivers, healthcare professionals, and other agency staff to coordinate and optimize client care.
Participate in staff meetings and training sessions to stay informed about agency policies, procedures, and best practices.
Administrative Duties:
Maintain accurate and up-to-date client records, including care plans, assessment reports, and visit documentation as per the department of health and agency standards.
Assist with scheduling and coordinating caregiver assignments to ensure adequate coverage and continuity of care.
Support the agency's compliance efforts by ensuring all client care activities adhere to relevant regulations and standards.
Qualifications:
Bachelor's degree in healthcare administration, social work, nursing, or a related field preferred.
Minimum of 2 years of experience in a homecare or other healthcare setting, with a focus on client care coordination.
Strong understanding of homecare services, including non-medical and concierge support.
Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and staff.
Strong organizational skills and attention to detail, with the ability to manage multiple tasks and priorities.
Proficiency in using electronic health records (EHR) and other client management software.
Working Conditions:
The position requires regular travel to clients' homes and other service locations.
Flexibility to work occasional evenings or weekends as needed to meet client needs and conduct supervisory visits.
The role may involve lifting, bending, and other physical activities related to client care and home visits.
