RN Clinic Manager St. Paul's PACE Reasner - Bankers Hill, 111 ELM STREET, SAN DIEGO, California, United States of America Req #2537 Thursday, January 30, 2025 San Diegos St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Pauls PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. This is a Full time position in our Reasner Clinic. The typical schedule is Monday - Friday, 8:00 AM - 4:30PM. Job Summary: Under the supervision of the Center Director, manages flow of clinic leading all staff in provision of medical and nursing care, ensuring high quality standards. Coordinates services with primary care team, acts as primary liaison. Triages, and assess participants as needed. Acts as gatekeeper for daily clinic scheduling. Provides skilled nursing care to participants, implements orders, develops appropriate plans of care, provides participant, caregiver, family, and staff teaching and communicates participants condition to caregivers, primary care, and the interdisciplinary team. To ensure continuity of operations assumes the Center Directors responsibilities in her/his absence Qualifications and Requirements: Education: Graduate of a school of professional nursing program required. BSRN strongly preferred. Current California Registered Nurses License and CPR certification required. Shall be at least 18 years of age. Experience: A minimum of 1 years experience working with a frail or elderly population required. Two years of nurse management experience in a hospital, nursing home or community-based setting preferred.
